-Drivers License or picture I.D.

-Social Security Card - Military I.D. Welcome

-Divorce Decree - if you have been married before and divorced within the last year.

Ages 18 to 21:

-Birth Certificate (NO COPIES)

-Drivers License or picture I.D.

-Social Security Card - Military I.D. Welcome

-Divorce Decree - if you have been married before and divorced within the last year.

Ages 16 and 17: (There is a five day waiting period)

-Parent or Guardian must accompany minor to sign marriage book.

-Guardians must provide proof of Guardianship.

-If Parents are divorced they must show proof of sole custody or both parents must accompany minor.

-Birth Certificate (NO COPIES)

-Drivers License or picture I.D.

-Social Security Card - Military I.D. Welcome

-Doctor note to verify pregnancy.
